Interestingly enough, Tommy himself wrote a huge chunk of this song about his own problem, but got no credit. JY had the beginning, and I think Dennis came on with some chord structure and melody, but the main body of the song had no words. Tommy wrote the lyrics to the parts he sang, those verses. One of many instances where Styx writing credits don't always reflect who did all of the work.
